1. application form (printed after you have filled it on-line)
2. transcript of records
3. Learning Agreement  (two copies)
4. a copy of the first page of the passport (with the photo and the number)
5. two photographs
6. a copy of a medical insurance
7. proof of English competence (for the students who do not study in English at their home institution) 
8. application for accommodation (please note that a single room is 440 PLN a month, a double is 390 PLN a month, you also pay a deposit of 350 PLN)
